Gift Voucher Terms and Conditions

§1 General Provisions

1. The issuer of Gift Vouchers is Premium Medical Services Sp. z o.o., ul. Adama Branickiego 10 lok. U6, 02-972 Warsaw, NIP: 9512522769, KRS: 0000910980, REGON: 389463419, hereinafter referred to as the Clinic.
2. These Terms and Conditions define the rules for the purchase and use of Gift Vouchers at the Clinic.
3. Purchasing a Voucher constitutes acceptance of the following terms.

§2 Voucher Purchase

1. Vouchers can be purchased at the Clinic reception and online on the Clinic's website.
2. The Clinic offers two types of Vouchers:
   • Value Voucher – a voucher for a predetermined amount (e.g., PLN 300, PLN 500) that can be used for any gynecological services and procedures available in the Clinic's offer. The Voucher value covers the full cost of the service, including medical materials, medications, and medical devices used during the visit (e.g., inserts, sutures, dressings used during the procedure). If the service cost exceeds the Voucher value, the Holder pays the difference.
   • Discount Voucher – a voucher providing a percentage discount (e.g., -10%, -20%) off the standard regular price of gynecological services and procedures. The discount applies only to the basic price of the procedure or medical consultation and does not include laboratory tests, medical materials, medications, or medical devices used during the service.
3. Vouchers are issued in two forms:
   • Registered Voucher – assigned to a specific person, requires presentation of a photo ID upon redemption.
   • Bearer Voucher – can be used by any holder.
4. Upon purchase, the Buyer receives a Voucher containing a unique verification code and expiration date.

§3 Returns and Withdrawal

1. A Voucher purchased at the Clinic reception is non-refundable due to its personalization.
2. A Voucher purchased online can be returned within 14 days of purchase without giving a reason. In this case, the Clinic will refund the full amount to the account from which payment was made, within 14 days of receiving the withdrawal statement.
3. The right to return does not apply if the Holder has already started using the services covered by the Voucher.

§4 Voucher Validity Period

1. The Voucher is valid for 6 months from the date of purchase.
2. The procedure or service must be performed within the validity period.
3. After the expiration date, the Voucher becomes invalid and can no longer be used.
4. The validity period cannot be extended, and unused Vouchers do not entitle to a refund.

§5 Voucher Redemption Rules

§6 Appointment Cancellation

1. The Holder can change or cancel the appointment by contacting the Clinic at least 24 hours before the scheduled time. Cancellation can be made by phone, in person, or by e-mail.
2. If the Holder cancels the visit later than 24 hours before the scheduled time, the Voucher is considered used. In this situation, the Holder loses the right to use the service again and is not entitled to a refund.
3. In exceptional situations, the Clinic may show flexibility – the final decision rests with the Clinic.

§7 Restrictions

1. The Voucher cannot be exchanged for cash or used to purchase another Gift Voucher.
2. Registered Vouchers are non-transferable to another person, while Bearer Vouchers can be used by anyone who possesses them.
3. The Clinic may refuse to honor a Voucher that is illegible, damaged, altered, or raises doubts about its authenticity.
4. Redemption will also be refused if the Voucher has already been used, has expired, was not previously booked, or the Clinic has no available appointments.
5. Any attempt to alter or counterfeit a Voucher renders it invalid.
6. In situations listed in paragraphs 3-5, the Clinic does not issue a replacement Voucher or refund.

§8 Loss or Theft

1. The Clinic is not responsible for lost, stolen, or destroyed Vouchers.
2. For Bearer Vouchers, there is no possibility of issuing a duplicate.
3. For Registered Vouchers, the Clinic may issue a duplicate upon presentation of ID and proof of purchase (receipt, invoice, or transaction number).

§9 Complaints

1. Complaints can be submitted in person at the Clinic reception, in writing to the registered address, or electronically to the Clinic's e-mail address.
2. The complaint should include the Complainant's contact details, the Voucher verification code, a detailed description of the problem, and – in case of complaints regarding service delivery – proof of purchase.
3. The Clinic will process the complaint within 14 business days of receipt and respond to the indicated e-mail address or mailing address.

§10 Personal Data Protection

1. The administrator of personal data is Premium Medical Services Sp. z o.o. based in Warsaw, ul. Adama Branickiego 10 lok. U6, 02-972 Warsaw.
2. The Clinic processes personal data for the following purposes: execution of the Voucher sale agreement, provision of medical services, maintenance of medical documentation (if the Holder uses services), fulfillment of legal obligations (accounting, taxes), and possible claims.
3. The legal basis for data processing is the execution of the agreement, legal obligation incumbent on the Administrator, and health service regulations (for patients using medical services).
4. Data subjects have the following rights: access to their data, correction, deletion (when permitted by law), restriction of processing, data portability, objection to processing, and filing a complaint with the President of the Personal Data Protection Office.
5. Detailed information is available in the Privacy Policy at the Clinic reception and on the website.

§11 Final Provisions

1. The Clinic reserves the right to change these Terms. Changes take effect from the date of publication on the Clinic's website and at the reception.
2. Changes to the Terms do not affect rights acquired from Vouchers purchased before the changes, unless the new terms are more favorable to the Holders.
3. The Terms are available at the Clinic reception and on the website.
4. The person purchasing the Voucher is obliged to inform the recipient about the rules of use, in particular about the validity period and the need for prior booking.
5. Any disputes arising from Voucher redemption shall be resolved by the common court having jurisdiction over the Clinic's registered office.
6. These Terms are effective from 08.12.2025.
